A stitch-like catch when you inhale, or a band of tightness between the shoulder blades, is often thoracic. Heart, lung and gallbladder pain can mimic this. The history — effort, breathlessness, sweating — is how we do not miss the dangerous list.
If the picture is mechanical I treat the thorax, ribs and neck as a unit. Breathing and desk height come into the plan. People are often surprised how much the ribcage affects neck headache.
Chest pain with breathlessness, collapse, or pain into the jaw or left arm is 999. New asthma-like symptoms or a cough with blood is a GP the same day.
